" Cold turkey " refers to the sudden cessation of substance dependence and unpleasant experiences produced, as opposed to gradually reducing the process through reduction over time or by using a replacement drug. The term is derived from piloerection or "goosebumps" which occurs with a sudden withdrawal from the opioid, which resembles cooled cooled turkey skin.

The sudden withdrawal of drugs such as alcohol, benzodiazepines, and barbiturates can be very dangerous, leading to potentially fatal seizures. For long-term alcoholics, being cold turkey can cause life-threatening delirium tremors, making this method inappropriate for stopping alcohol addiction.

In the case of withdrawal of opioids, "cold turkey" is very unpleasant but less harmful. A life-threatening problem is not possible unless a person has a pre-existing medical condition.

The smoking cessation method was advanced by J. Wayne McFarland and Elman J. Folkenburg (an MD and a minister who wrote their Five Day Plan around 1959), Joel Spitzer and John R. Polito (educator quitting smoking) and Allen Carr (who founded Easyway during early 1980s) is a cold turkey plan.

Etymology

The initial use of printed terms from 1920:

Some addicts voluntarily stop taking opiates and "suffer" because they reveal it without medical help, a process in their slang term called "cold turkey"...

The use of other printed terms in the media to refer to drug withdrawal occurred at Colonist Daily in British Columbia in 1921:

Probably the saddest figure that appeared in front of him. Carleton Simon... are those who voluntarily surrender. When they left before him, who was given the so-called 'cold turkey' treatment.

The phrase "taking cold turkey" has also been reported during the 1920s as a slang for pleading guilty.


The term is then seen in the 1947 novel I, The Jury "by Mickey Spillane:

Included was a medical record from the hospital when he made it into a cold turkey, which is a drug addict to heal everything.

On February 26, 1951 Time "High & amp; Light" magazine article using the phrase, states:

There is one side of hope that is somewhat grim on teenage drug problems. Unlike parents, some teenagers seem to use drugs because of psychological problems; children usually start using narcotics either because of the same ignorance or impetuous drive that leads them to race hot rods. Although they are more easily weaned, but almost no facilities to treat it. On New York City's Rikers Island, children must bear the horror of a sudden "turkey cold" or not get it at all. Once released, many are returning to drugs again.

In the early drug hose, the term "being cold turkey" was referred to stop the drugs suddenly and totally. This term is now used in a common slang with a broader meaning to stop any habit or any exercise suddenly.

There are several explanations about the origin of the phrase:

  • The "goose bumps" (piloerection) that occur with the sudden cessation of opioid-dependent people, are often addicted to heroin. Similarly the term "kicking habits" alludes to muscle spasms that also occur.
  • Refinement of meaning "suddenly or without preparation", from cold turkey to a dish that requires little preparation; originally used for heroin addicts.
  • From the American phrase talk turkey which means "speaking frankly with a little preparation".
